On the evening of September 19, 2014, tragedy struck aboard the cargo vessel 'Silver Pegasus' as a Vietnamese crew member fell into the ship's cargo hold while docked in Brake. The fall resulted in catastrophic injuries, and he later succumbed to them at a hospital in Oldenburg. At the time of the incident, the 2nd officer was conducting a survey of operations while the ship was in the process of unloading approximately one thousand tons of cargo. Preliminary reports suggest that the crew member may have slipped from a ladder, plunging six meters down into the hold. Other crew members discovered him unresponsive and promptly called for fire rescue services, who utilized a stretcher to extricate him from the hold. Following the incident, the vessel underwent a brief detention for a thorough investigation by local police, but once it was established that there were no lingering safety concerns, the 'Silver Pegasus' departed for Macapa on September 20.
