The 24 year old, Indian Third Officer of the 'Shandong Zhe' was found dead in his cabin on Oct 1, 2021, while thetanker was en route to the Banco Del Hoyo Anchorage W inside the Gibraltar Strait, coming from ports of Gulf of Guinea. He didn’t show up to replace CO for dinner, and the general alarm was sounded and the crew mustered for his search. At 7 p.m. he was found hanging in bow thruster room,. A cardiac resuscitation CPR was applied, later he was transferred to an infirmary, still alive, but at 8.45 p.m. UTC he was declared dead by the Telemedical Maritime Assistance Service TMAS, which had been called immediately after he was found. The tanker arrived at the Europa Point Gibraltar on Oct 3, and remained stationary as of Oct 4. The officer had boarded the tanker at the end of August in Lome,

The "Shandong Zihe" wich had suffered a technical failure south of Canakkale on April 18, was still under repair at the Yalova Shipyard in Tuzla on May 13. Final tests and sea trials were to be carried out after completion of repairs. On June 28 she left the yard, bound to Novorossyisk.

The "Shandong Zihe", en route El Dekheila,to Tuzla,, suffered a technical failure south of Canakkale on April 19, 2019. The tug "Ocean Ergun" was appointed to tow the vessel to Tuzla, ETA April 21. During the Canakkale Strait passage, a Coastal Safety tug provided escort service for vessel and strait traffic safety. The ship arrived at Diliskelesi on April 23 at noon. After the completion of formalities it entered the Yalova shipyard for repairs the same day.
