Destroyer commander removed from his ship after collision
The commander of the USS 'Truxtun' was removed from the ship on Feb 22, due to a loss of confidence in his ability to command, following the ship colliding with the USNS 'Supply' The head of Naval Forces Southern Command, Rear Adm. Carlos Sardiello, relieved Cmdr. James Koffi of command of the Truxtun. Cmdr. Taylor Auclair, previously with the U.S. Fleet Forces Command was named the interim commander of the Arleigh Burke-class destroyer. Koffi has been reassigned to Commander, Naval Surface Group Middle Atlantic. The USS 'Truxtun' was only a few weeks into its deployment to the Caribbean, having left the Naval Station Norfolk, when the collision occurred while the ship was being replenished at sea by the oil supply vessel, after veering to starboard, hitting the oiler. The crash remained under investigation.

The destroyer USS 'Truxtun-DDG-103' (MMSI: 303882000) and the USNS 'Supply-T-AOE-6' were in collision during a replenishment-at-sea operation in the Caribbean Sea on the afternoon of Feb 10, 2026. Two personnel suffered minor injuries and were reported to be in stable condition. Both vessels were able to continue sailing following the incident, which was under investigation. The collision occurred just over a week after the 'Truxtun' sailed from the Naval Station Norfolk on Feb 3 for a scheduled deployment. The destroyer carried a crew of roughly 300 sailors, along with 26 embarked air wing personnel assigned to Helicopter Maritime Strike Squadron 50, known as the “Valkyries.” The supply ship suffered hull damage and breaches above the water line at port side and berthed in Ponce, Puerto Rico, on Feb 16. The collision occurred just over a week after the 'Truxtun' had sailed from the Naval Station Norfolk, Virginia, on Feb 3 for a scheduled deployment. The destroyer carried a crew of roughly 300 sailors, along with 26 embarked air wing personnel assigned to Helicopter Maritime Strike Squadron 50, known as the “Valkyries.” The ship subsequently returned to port for repairs to an unspecified piece of equipment, but was underway again by Feb 6. The 'Supply' had departed from Ponce, Puerto Rico on Feb11, the day of the collision, after arriving there two days earlier. The vessel had previously conducted a replenishment-at-sea with the Ticonderoga-class cruiser USS 'Gettysburg' on Jan 29 as part of Southern Command operations focused on counter-narcotics and regional security in the Caribbean.
